ABSTRACT

Objective To establish the theoretical basis of the probiotic application among very low birth weight(VLBW) infants,the efficacy of probiotics on the gut microbiota of VLBW infants on the 14th postnatal day was studied.Method The VLBW infants admitted to BaoAn Maternal and Child Care Hospital from January 2015 to December 2015 were randomly assigned into probiotics group and placebo group.From the first feeding to corrected gestational age of 36 weeks,probiotics group was treated with a combination of Bifidobacterium and Lactobacillus while placebo group with placebo.Fecal samples were collected on the 1st and 14th postnatal day.Total bacterial DNA was extracted and sequenced using high-throughput 16S rRNA gene sequencing on MiSeq sequencing platform.Result A total of 21 VLBW infants were enrolled,9 in probiotics group and other 12 placebo group.No significant differences of clinical data existed between the two group (P > 0.05),The abundance and diversity of microflora (P > 0.05) on the first day between the two group were similar.Compared with placebo group,the relative abundance of Bifidobacterium and Lactobacillales in stool samples on the 14th day was significantly increased,while the Halomonas was significantly decreased.The relative abundance of the Shannon-index was increased,but without significant difference (P =0.16).Conclusion Enteral supplementation of probiotics in VLBW infants may increase probiotic bacterium and microflora diversity.

ABSTRACT

Objective To investigate the clinical effects of peroral endoscopic myotomy (POEM) in the treatment of achalasia of cardia. Methods Clinical data of 27 cases who were diagnosed with achalasia of cardia recived POEM from May 2014 to March 2016 were collected retrospectively. The surgical results, before and after POEM, parameters measured by esophageal manometry and complications after POEM and during follow-up were analyzed. Results POEM were successful 100.0% in the 27 patients. There was 1 case of subcutaneous emphysema, 1 case of aeropleura symptoms were significantly improved in all patients who had successful POEM; parameters measured by esophageal manometry were also improved obviously (P < 0.05). Conclusion POEM has appreciable short-term effects in the treatment for achalasia of cardia, and it can be relieve dysphagia and other adverse symptoms in postoperation, but the long-term efficacy and complications need further follow-up observations.

ABSTRACT

OBJECTIVE:To compare the effects of different doses of ramipril on blood pressure and renal function of early di-abetic nephropathy patients. METHODS:A total of 108 patients with early diabetic nephropathy selected from our hospital during May 2014-Dec. 2015 as research objects were divided into group A,B,C,with 36 cases in each group. All groups were given Ramipril tablets,5 mg for group A,2.5 mg for group B,1.25 mg for group C,qd,for 8 weeks. Blood pressure(SBP,DBP)and renal function indexes(24 h urine protein,β 2-microglobulin,UAE)were compared among 3 groups before and after treatment. The occurrence of ADR was also recorded in 3 groups. RESULTS:Before treatment,there was no statistical significance in blood pressure or renal function indexes among 3 groups(P>0.05). After treatment,blood pressures 24 h urine protein,β2-microglobulin and UAE of 3 groups were decreased significantly;those indexes of group A were significantly lower than group B and C,with sta-tistical significance(P0.05). Both hypoten-sion and hypoglycemia occurred in 3 groups during treatment,but were recovered after stopping treatment. CONCLUSIONS:High dose of ramipril(5 mg,qd)shows good effects on early diabetic nephropathy and reduces urinary protein excretion,but doesn't af-fect its safety.

ABSTRACT

Objective To evaluate the effectiveness of intratracheal injection of pulmonary surfactant (LISA) combined with continuous positive airway pressure (CPAP) on the treatment of neonatal respiratory distress syndrome (RDS). Methods The databases of PubMed, EMBASE, Cochrane, China National Knowledge Infrastructure (CNKI), Wanfang Database, Weipu Database and China Biology Medicine disc (CBM) were searched, and all of the randomized controlled trial of LISA combined with CPAP for treatment of RDS were included. RevMan 5.2 was used to perform meta-analysis. The differences of mortality, bronchopulmonary dysplasia (BPD), retinopathy of prematurity (ROP), intraventricular hemorrhage (IVH), periventricular leukomalacia (PVL), and other complications between LISA group and control group were compared in 0.05). Conclusion The mechanical ventilation and the incidence of BPD is significantly reduced in the treatment of neonatal respiratory distress syndrome by LISA, but it has no effect on the incidences of other complications and mortality.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the anti-inflammatory and analgesia mechanism of electroacupuncture (EA) device of point injection (PI) on rats of inflammatory pain.</p><p><b>METHODS</b>48 Sprague Dawley (SD) rats were randomly assigned into a control group, a model group, an EA+PI group, an EA device of PI (EAPI) group, an EA group and a PI group, eight rats in each one. The rats in the control group were subcutaneously injected with 50 μL of liquid paraffin oil solvent into the dorsum of left hindpaw, while rats in the remaining groups were treated with 50 μL of complete freund's adjuvant (CFA) at identical location to induce the model of inflammatory pain. After model establishment, the rats in the EA+PI group, EAPI group, EA group and PI group were treated with EA+PI,EA device of PI, EA and PI, respectively, once every other day (the 2nd day, 4th day and 6th day). Each treatment was given for 30 min. The mechanical withdrawal threshold, thermal withdrawal threshold and foot swelling before and 1 d to 6 d after model establishment were observed; the western blotting method was applied to measure IL-1β expression in inflammatory tissue of skin.</p><p><b>RESULTS</b>After model establishment, compared with the control group, the mechanical withdrawal threshold and thermal withdrawal threshold were reduced (all<0.05) and the foot swelling was increased in the rest groups (all<0.05). After treatment, the mechanical withdrawal threshold and thermal withdrawal threshold in the EAPI group were significantly increased compared with those in the EA+PI group, EA group and PI group (all<0.05), but the foot swelling was reduced (all<0.05). The IL-1β expression in the model group was higher than that in the control group (<0.05); after treatment, the IL-1β expression in the EAPI group was lower than that in the model group, EA group and PI group (all<0.05), but no significantly different from that in the EA+PI group (>0.05).</p><p><b>CONCLUSIONS</b>The efficacy of EA device of PI on inflammatory pain is superior to EA combined with PI, EA alone and PI alone, which is suitable for further popularization and application.</p>

ABSTRACT

OBJECTIVE:To investigate the effects of Astragalus injection combined with Salmeterol propionate powder inhala-tion on pulmonary function,cytokines and blood rheology indexes of Chronic obsrnctine pulmonary disease (COPD) patients. METHODS:104 COPD patients were divided into observation group and control group according to the randorn number table meth-od,with 52 cases in each group. Control group was given routine symptomatic treatment+Salmeterol propionate powder inhalation 1 dose,bid;observation group was additionally given Astragalus injection 30 ml added into 5% Glucose injection 250 ml,ivgtt, qd,on the basis of control group. Treatment course of 2 groups lasted for 2 weeks. Clinical efficacy,pulmonary function indexes [forced expiratory volume in one second(FEV1),forced vital capacity(FVC),FEV1/FVC],CRP,cytokine [IL-6,IL-8,TNF-α] levels,blood rheology indexes (whole blood high-shear viscosity,whole blood low-shear viscosity,fibrinogen,hematokrit and plasma viscosity),and the occurrence of ADR were observed in 2 groups. RESULTS:The total effective rate of observation group (94.23%)was significantly higher than that of control group(75.00%);compared with before treatment,FEV1,FVC and FEV1/FVC of 2 groups were significantly increased after treatment,and those of observation group were significantly higher than those of control group;CRP,IL-6,IL-8 and TNF-α of 2 groups were significantly decreased,and those of observation group were signifi-cantly lower than those of control group. The whole blood high-shear viscosity,whole blood low-shear viscosity,fibrinogen,hema-tocrit and plasma viscosity of observation group were decreased significantly after treatment,and the observation group was signifi-cantly lower than the control group,with statistical significance(P<0.05). No significant ADR was found in 2 groups. CONCLU-SIONS:Astragalus injection combined with Salmeterol propionate powder inhalation is significantly effective for COPD,improves pulmonary function of patients,improves micro inflammatory state and decreases blood rheology indexes with good safety.

ABSTRACT

Objective To discuss the effect of the modified structure of oxygen equipment on oxygen inhalation effect of patients receiving oxygen therapy.Methods The structure of oxygen equipment was modified.and 80 patients who required oxygen therapy were divided into the modified group and the non-modified group according to odd and even number of hospital admission sequence randomly with 40 patients in each group.The modified group used the modified oxygen equipment,the non-modified group used traditional one.The differences of respiration,heart rate and blood oxygen saturation rate between the two groups before oxygen inhalation,30 minutes and 2 hours after oxygen were compared.Results There was no significant difference in the respiration,heart rate and blood oxygen saturation rate between the two groups.Conclusions The modified oxygen equipment doesn't affect the oxygen therapy effect of patients,besides,it avoids the problems of pollution and the complicated disinfection procedure of the structure of the humidifier bottle,fluid and breathing vessel of the traditional equipment.

ABSTRACT

OBJECTIVE:To investigate the efficacy of compound azintamide enteric-coated tablet plus mosapride citrate dispersible tablet for abdominal distention in patients with functional dyspepsia(FD).METHODS:A total of 80 patients with the chief complaint of abdominal distention were randomized to treatment group(Group A,n=40) to be treated with mosapride citrate dispersed tablets plus compound azintamide enteric-coated tablet,or control group in which the patients were treated bu mosapride citrate dispersed tablets(Group B,n=20) or compound azintamide enteric-coated tablets(Group C,n=20).Comp-ound azintamide enteric-coated tablets were administered orally tid(2 tablets) immediately after meal while the mosapride citrate dispersed tablets(5mg) were administered orally tid 30min before meal for 2 weeks.The main outcome measures were changes of symptom scores of abdominal distension,the total effective rates in improvement of abdominal distension,and drug side effects before and after treatment.RESULTS:After 1-week and 2-week treatment,the average index scores of abdominal distention in the treatment group(Group A) were significantly lower than in the control groups(Group B and Group C) (P
